Origin Wellness Affiliate Agreement

Between: Origin Wellness (“Company”) and Registering Affiliate (“Affiliate”)

1. Program Overview

Origin Wellness is a medically supervised telehealth platform offering compounded GLP-1 medications (Tirzepatide, Semaglutide) and premium non-GLP-1 peptides. Every program includes access to diagnostic lab testing and oversight by licensed medical providers.

2. Compensation & Discounts



A. Affiliate Commission

• $25 flat commission per successful paid referral on any program (GLP-1 or non-GLP-1).
• A “successful referral” is a new customer who completes a paid program through your unique referral link or code and does not request a refund or chargeback.
• Commissions are paid monthly via ACH once the $25 minimum threshold is reached.

B. Customer Discounts (Promotable to Your Audience)

• $25 off the first program for all first-time customers who use your affiliate link or code.

C. Affiliate Wholesale Discount (Personal Use Only)

• Approved affiliates receive 10% off all personal orders.
• This discount is for your personal use only and may not be shared publicly with your audience or used for family and friends.
• The code will be restricted to your affiliate-tagged account.

3. Tracking & Payouts

• You will receive a coupon code upon approval.
• Access to your real-time Affiliatly dashboard for tracking referrals and earnings.

4. Affiliate Obligations & Compliance

• Always disclose your affiliate relationship (#ad or #affiliate).
• You may not prescribe, recommend specific treatments, make medical claims, or guarantee results.
• Do not present yourself as an employer or licensed provider for Origin Wellness.
• All promotional content must follow the Approved Messaging Guide (found in our FAQs in your Affiliatly dashboard).

5. Term & Termination

• Month-to-month agreement.
• Either party may terminate with 30 days’ notice or immediately for breach.
• Commissions on already-completed referrals will still be paid.

6. Intellectual Property, Confidentiality & Indemnification

• Limited license to use Origin Wellness trademarks and materials for promotion only.
• You agree to indemnify the Company against claims arising from your marketing activities.

7. Governing Law

This agreement is governed by the laws of the State of Texas.
